PEMBERIAN EDUKASI CONTINUITY OF CARE (COC) PADA IBU ‘NA’ USIA 32 TAHUN SEJAK KEHAMILAN HINGGA MASA NIFAS

Pregnancy is a physiological process that requires special attention, especially for Primigravida mothers who have a higher risk of obstetric complications. This case study aims to determine the results of the implementation of Continuity Of Care midwifery care for a 29-year-old primigravida mother "NI" and her baby who received comprehensive and continuous midwifery care from 16 weeks of pregnancy to 42 weeks of postpartum. The methods used were interviews, examinations, observations and documentation. Care was carried out from October 2024 to May 2025. The pregnancy period was physiological and the complementary care provided during pregnancy was a brain booster with Mozart classical music, prenatal yoga, massage efleurage and Gym Ball. Giving birth at full term vaginally without complications. The baby was born spontaneously, birth weight 2800 grams and Early Breastfeeding Initiation was carried out. The postpartum period was physiological, care was given according to standards and the complementary care provided was oxytocin massage. The lactation process is not hampered because it is supported by the husband and family. Midwifery care for babies is in accordance with essential neonatal services and complementary baby massage is carried out. Through continuous care, pregnant women are more prepared and confident in facing every physiological change. Risk factors can be detected as early as possible so that no complications occur in the mother or baby.
